By identifying potential health risks and giving you suggestions to minimize them, we help you take a more active role in maintaining good health. Participants in Optimal Health have shown tremendous enthusiasm for the program, citing the satisfaction they find in setting and reaching their own health improvement goals. They also respond especially well to the fact that Prairie States performs it all in-house.

After you have answered a series of questions, a health risk score is calculated to help identify possible opportunities for lifestyle improvement. Members enrolled in the Optimal Health Wellness Program receive individualized follow-up assistance for maintaining or improving health behaviors through coaching, education and medical self-care practices.
