True Story #1 Health Coach Saves the Day

A Condition Management member was on the phone with a Prairie States coach and described feeling “funny." After a few questions the coach quickly ascertained that the member was new to a powerful insulin, U-500, but had not received any diabetes education with respect to low blood sugar signs, symptoms and treatment.
The coach inquired about a blood sugar meter that the member had and requested the member test while she remained on the phone. The low sugar was severe enough that this would have resulted in a 911 call within a short time. Instead, the coach talked the member through treatment and the member stabilized.
Remarkable Result for the Member and the Bottom Line:
Emergency call for an ambulance and an emergency room visit was avoided. The member was educated about symptoms and medication usage. Two years later, the member was in good diabetes control and on an insulin pump.
